Polk County Bank collects the following categories of nonpublic personal information about our consumer customers:

  • Information received from you on applications and other forms such as your name, address, social security number, assets and income.
  • Information about transactions with us such as: information about a deposit or loan account, payment history, parties to transactions and credit/debit card usage.
  • Information we receive from consumer reporting agencies such as your credit worthiness and credit history.

Confidentiality and Security of Nonpublic Personal Information

Ensuring your financial privacy is of vital importance to us. Polk County Bank maintains the following policies and practices designed to protect the confidentiality and security of consumers’ nonpublic personal information:

  • We limit employee access to confidential customer financial information to those employees with a business reason for knowing such information.
  • Our employees are trained to understand the importance of customer financial privacy and to properly handle confidential information.
  • We maintain physical, electronic, and procedural safeguards that comply with federal standards to guard your non-public personal information.

Information We Share with Others

Affiliates-An affiliate is a company we own or control, a company that owns or controls us, or a company that is owned or controlled by the same company that owns or controls us. Ownership does not mean complete ownership, but means owning enough to have control. Our affiliate includes Polk County Bank Investment Center. We may disclose information about you to our affiliates. This may include information about your transactions and experience with us such as: Name, Address, Account Balances, Account Activity, Types of Accounts, Credit Card Usage, Payment History, Deposit History. Customers have the opportunity to opt out of information derived from other sources such as the application information including social security number, asset and income.

Joint Marketing-We may disclose information we collect about you as described above, to companies that perform marketing services on our behalf with whom we have joint marketing agreements containing privacy disclosures.

Others-We report account information such as credit limit, balances and payment information to credit bureaus.

Disclosures to Nonaffiliated Third Parties

We will not disclose information about you to anyone except as disclosed in this policy or as permitted by law.


This policy applies to former customers as well as current customers.
