Definitions: Throughout this policy statement, "We, our, and us" means Polk
County Bank. "You and yours"; means anyone who uses bank services in any way,
including those who have applied for bank services and been denied. "Publicly
available information" means information that is lawfully available to the
general public from official records (such as real estate records and securities
filings), information from widely distributed media (such as phone books,
television, radio or newspaper) or information that is required to be disclosed
to the general public by federal, state or local law (such as securities
filings). "Nonpublic personal information" means personally identifiable
financial information that is not available from public sources (as described
above). "Affiliate" means any company under common control with another
company. "Consumer Reporting Agency" means a person or business which makes a practice of collecting and evaluating consumer credit information for the purpose of furnishing consumer reports to third parties.
CONFIDENTIALITY AND SECURITY OF NONPUBLIC PERSONAL INFORMATION

INFORMATION WE SHARE WITH OTHERS
Affiliates – An affiliate is a company we own or control, a company that owns or controls us, or a company that is owned or controlled by the same company that owns or controls us. Ownership does not mean complete ownership, but means owning enough to have control. Our affiliate includes Polk County Bank Investment Center. We may disclose information about you to our affiliates. This may include information about your transactions and experience with us such as: name, address, account balances, account activity, types of accounts, credit card usage, payment history, and deposit history. Customers have the opportunity to opt out of information derived from other sources such as the application information including social security number, assets, and income.
Joint Marketing – We may disclose information we collect about you as described above, to companies that perform marketing services on our behalf with whom we have joint marketing agreements containing privacy disclosures.
Others – We report account information such as credit limit, balances and payment information to credit bureaus.
Disclosures to Nonaffiliated Third Parties – We will not disclose information about you to anyone except as disclosed in this policy or as permitted by law.
This policy applies to former customers as well as current customers.
